Hospitals hold meetings where the worst outcomes get walked through in detail, out loud, in front of colleagues. No blame and no burial. Just what happened and what changes.

Alone, we tend to do the opposite. Bury the mistake, feel terrible at three in the morning, extract almost nothing usable from it.

Shame is the thing that blocks the lesson. It makes you look away at exactly the place you'd need to look closely. Embracing them isn't pretending they cost nothing. It's holding one long enough to read it, which is the only way it becomes anything other than a bruise.
